Prohibitions against saying name of God?

What do we really know about prohibitions against saying the name of God in Judaism? When did this actually start? What was the basis for this position? How was it enforced? Were these actual laws or just customs? Was this adopted by all Jewish sects? Etc., etc.

I'll note one here.

In Community Rule (1QS), Col 6:27 - Col 7:2:

Anyone who speaks aloud the Most Holy Name of God, [whether in ...] or in cursing or as a blurt in time of trial or for any other reason, or while he is reading a book or praying, is to be expelled, never again to return to the party of Yahad.
