Theophilus was the high priest at the time of Luke's Gospel, 41 AD, so he was "most excellent". He did not have that moniker when Acts was written, c. 62 AD.

Actually, Theophilus is the scantily-clad youth in Mark 14:51.

Luke was very likely one of the great company of priests.
Acts 6:7
And the word of God increased; and the number of the disciples multiplied in Jerusalem greatly;
and a great company of the priests were obedient to the faith.
The person was identified as a physician because he was a different Luke.
